Our Case Management services are a cornerstone of our reentry efforts. Each participant is paired with a dedicated case manager who works closely with them to create a personalized plan for success, ensuring they have the guidance, accountability, and support they need throughout their journey. From legal aid to healthcare navigation and goal-setting, case management ensures that every individual has the resources to overcome obstacles and build a new life.
